2023 Championship Application Deadline/Form

2023 Championships need to be completed and sent to the Provincial UIC by December 31, 2022.

Applications should NOT go directly to the Provincial UIC.  They need to be submitted by your Branch President with their comments.

Each branch may have their own deadline for submission in order to reach the provincial deadline.

The evaluator for Level III has been set at U17 level.  Please remember to get any evaluations along with the application form.  Zone’s should be reviewing the application to see if the official is the right level to officiate at the events selected.
